CV Ice Hockey Club:
Following are the dates for the 2011-2012 Player Evaluations. It is important that ALL players attend ALL
sessions for their level. If you can not make an evaluation for whatever reason, please email Patti,
enck@comcast.net.
Please note: All evaluations will take place at Twin Ponds West. PLEASE PLAN TO ARRIVE EARLY SO
YOU HAVE TIME TO REGISTER BEFORE TAKING THE ICE.
To assist with the evaluation process, Coach Lombardi would like all players to wear the following
jerseys at each evaluation:
Last name beginning with A - L: wear WHITE jersey
Last name beginning with M - Z: wear RED jersey
HIGH SCHOOL (VARSITY/JV) EVALUATIONS: (grades 9 through 12)
Monday, August 22nd from 7:00 - 7:50
Wednesday, August 24th from 6:00 - 6:50
MIDDLE SCHOOL EVALUATIONS: (grades 6 through 8)
Wednesday, August 24th from 4:00 - 4:50
All REGISTRATION PAPERWORK MUST BE COMPLETED before your child will be allowed to participate
in the evaluations.
This includes:

CVIHC Waiver of Liability Form
CVIHC Player Code of Conduct
CVIHC Parent Code of Conduct
CVIHC Policy #227: Random Drug Testing/Breathalyzer Form
USA Hockey Consent to Treat/Medical History Form
USA Hockey Waiver of Liability Form
USA Hockey Participant Code of Conduct
Copy of Birth Certificate or Drivers License
Copy of USA Hockey Registration w/barcode (completed on-line at www.usahockey.com)
$200.00 non-refundable registration fee (This registration fee includes evaluation fees and payment
towards season fees)
You are invited to pre-register in advance of the evaluations, by sending all completed paperwork and
